Bottom line: This is a tool I developed to help our interns and residents in approaching their toolkit in end-of-life symptom management and prognostication. It is based on clinical experience and incorporates aspects of evidence from the listed references. It is structured to be best printed out (double-sided), such that each printing provides two copies.
